What We Do

We offer care leavers and young unaccompanied minors (16-21) accommodation and support. We aim to prepare young people to live independently, by providing the right environment and support. We help young people with education, health, financial and immigration issues. We have keyworkers who accompany young people to appointments to provide support and give professional advice.

Without proper support at a crucial time in life, young people can enter adult life with many disadvantages. This is especially true for those leaving care, or separated from their family or cares. Home Hub Supported Living provide tailored interventions to meet these challenges, ensuring the young people are equipped for adult life. Forging relationships with local authorities is key to our work since by working together we can give the young person the skills they need for independent living.

We offer placements for young people leaving care on the way to becoming independent adults. This is much more than just accommodation, we offer a complete package of care. Our experienced staff work closely with each young person to identify their specific needs, building trust, teaching them vital life skills and self-empowerment.

We collaborate with each young person to create a tailored care plan, working closely with parents/guardians, social workers, and any other professionals involved in their care. This is a youth-centred service, empowering the young people to be the best they can be. Each young person is assigned a key worker with who they can immediately start working.

It is vital to track the progress of each young person, so appropriate changes can be made as needed. To achieve this we use a range of self-assessment tools.

Identifying cultural needs and ensuring they are met.

Self-Empowerment (ensuring the young person is involved in their care and that their voice is heard).

Promoting good health through learning how to eat healthily on a limited budget.

Our service is tailored to each individual. For example, some of the young people we work with are claiming asylum they often lack language skills and understanding of UK life. So we offer these individuals additional support to make sure they have the skills they need. We can help provide all sorts of vital skills: cooking, cleaning, budgeting, travel, and so on. Our aim is for them to eventually gain all the skills needed to live an independent and meaningful life.
